Welcome to Bridge Assistive Technology!
Bridge will provide up to $1800 of assistive technology (AT) (low or high tech) to individuals with disabilities living on Nisqually Indian Tribal Land and in Lewis, Mason, and Grays Harbor Counties. Individuals must live in a low- to moderate- income household and have a documented disability.
Please complete the intake form to begin the process. Documentation for eligibility is required for the Bridge program. You can upload documents as part of your intake form or email them to BridgeSupport@SensoryToolHouse.com.

You will be required to provide documentation of your place of residence, household income, and disability to qualify. Examples of the types of documentation that will be accepted include:
Residency
- A dated utility bill which includes your name and address
- Current WA State ID
- A tenancy agreement with your full name and all the information about how long you have lived in your current address
- Tax forms or tax returns
- Military papers
- Vehicle registration card or title
- Mortgage or lease papers
- Property tax statement
Household Income (for all earners of the household)
- Pay Stubs: Show your earnings from an employer, including gross pay and deductions.
- Tax Returns: Federal and state tax returns detail your annual income.
- W-2 Forms: Issued by employers annually, showing total earnings and taxes withheld.
- 1099 Forms: For freelance or contract work, dividends, or interest.
- Social Security Benefits Statements: For those receiving social security income.
- Unemployment Benefits Statements: For individuals receiving unemployment compensation.
- Worker’s Compensation Letters: Document income received through worker’s compensation.
- Court-ordered Agreements: Alimony or child support documents.
Disability
- 504 Plan or IEP
- Care Plan from DDA
- Medical Provider Documentation
- Other Documentation by a Qualified Provider
